Cerebra OTel Mapping Aligned to Locked Coordination Spec
Four post-rc.1 corrections to §8.2 of AGENT_TRACE_VOCABULARY.md align the OTel GenAI span mapping to the decisions locked in the Cerebra coordination round before the exporter ships.

·C3: Option A span shape applied — 10 rows corrected from INTERNAL (single span) to span events on step span; §8.2 opening paragraph rewritten to describe the session→cycle→step hierarchy and Option A explicitly
·C4: StepExecutionFailed added to mapping table after StepExecuted (sets step span status to error)
·C5: §2 cross-reference (§7.3.2 note for ContextPacketBuilt overlap) verified correct; no change needed
